If you are doing a SOD analys for E-Business application, how much necessary is to go down the database and OS and do also a security analysis?

I'm trying to understand the considerations to take regarding OS and database security, so you could be sure that your SOD analysis is complete and accrued or if those (OS and database) are only taken in count for PEN tests.

Are there some security problems at database level that could lead to a unauthorized modification on the access and SOD controls implemented at application level?
